            Inspired by the life and work of Mother Teresa, Aging with Dignity works to affirm the dignity of persons as they age and to promote compassionate care for those nearing the end of their life.  Aging with Dignity created Five Wishes as a tool that addresses spiritual and emotional aspects of end of life care planning as well as medical and legal aspects.  Five Wishes is legally recognized in 42 states and is available in 27 languages and in Braille.

            The Starter Kit provides a thoughtful process to have the conversation with loved ones about end-of-life wishes.  The Starter Kit--available in English, Spanish, French, and Mandarin—can be completed online, saved, and downloaded.  Or it can be downloaded as is and completed (see below for downloadable link).
