The Faith and Community Health Network(FCHN) is a pilot program that brings together community services, healthcare facilities and faith communities. With the focus on mind, body, spirit and community, ICS will partner with faith communities to develop volunteer teams that will provide non-medical assistance to patients at high risk for hospitalization readmission. By bridging the faith communities with healthcare facilities and connecting to essential community resources, together we can support at-risk and vulnerable patients during and following discharge from the hospital.

Participating Healthcare Facilities
FCHN Pilot Project is with Banner-University Medical Center Tucson Campus
FCHN plans to expand to other healthcare facilities in Tucson in 2016
